Athlete Registration

Please fill and turn in the following to complete your Registration (Steps 1-9):

1) Registration Forms due Feb 22:

What you will find in the Athletic Package for Lacrosse includes:
All forms due February 22, 2012. If you do not turn in your forms, you will not be allowed to practice or play Lacrosse.

  • Parent Permission
  • Physical Evaluation/Exam
  • Injury option
  • Emergency Information Form.
  • Transfer Form

2) 205 - Regular Season Player Fee $75 Due by Feb 28

-You also need to pay this along with your registration papers. You may also submit a check no later then Feb 28 and hand it to Coach Brin.

3) Varsity Tryouts 2012 Feb 27 thru 29

Feb 27 thru 29 at 3:25 with all required equipment on and ready to go.

4) Practice Starts on Feb 27, 2012 at 3:25

for all players with all required equipment on and ready to go. Attend the first day of Practice Monday February 27th.

Remainder: In order to play Lacrosse this season the following should have been submitted:

  • Mullen High School Athletic Registration, Emergency Card, and Physical Evaluation/Exam. Due before the Feb 22
  • Athlete Code of Conduct but it can be turn in no later then March 1.
  • Copy of Emergency Card to your Coach. Due Before the Feb 29
  • Players Season Fee due before Feb 29
  • Team Shorts due by Feb 29 and turn in at the Face-Off Dinner. We will hold your check for the Varsity Team shorts payment until we know who made it.

5) Sign and Turn in your Athlete Code of Conduct 2012 Due by March 1.

The download Document is found at the bottom of the page. Please click on the attachment. Sign. Turn it in to Coach Brin early and no later then by March 1, 2012.

6) Sign and Turn in a copy of your Emergency Card to the Coach or Diana Nelson at the Face-Off Dinner due by Feb 29.

A copy can be obtain from the Athletic Secretary.

7) Team Shorts Purchase Due by Feb 29

-Once you find which Team you are on, pay either by Check (pay at the Face-Off) or online (also found under Shop-Equipment) for the corresponding Team shorts due Feb 29 as follows:

8) Attend the Face-Off Dinner Feb 29:
Feb 29 at the Rilko Center
